This protocol is suitable for patients with a new mechanical valve or new LVAD.
No initial bolus is given to patients with new mechanical valve or new LVAD day 1.
Initial dose is 12 units/kg/hr to a maximum of 1000 units/hr, calculated in Epic.
Day 1 goal is aPTT is 40-60 seconds.
Day 2 and beyond goal is aPTT is 53-71 seconds.
The first aPTT is obtained 6 hours after initial infusion is started.
After day 1, if 2 consecutive aPTTs are withbetween 53-71 seconds, repeat aPTT in am daily.
Notify physician for 2 consecutive aPTTs < 53 or > 71 seconds.
