This protocol is for patients on VV ECMO.
Initial bolus is 60 units/kg to a maximumm of 4,000 units, calculated in Epic.
Initial dose is 12 units/kg/hour to a maximum of 1,000 units/hr, calculated in Epic.
Subsequent goal aPTT is 40-60 seconds.
The first aPTT is obtained 6 hours after initial infusion is started.
Once 2 consecutive aPTTs are between 40-60 seconds, repeat aPTT in am daily.
Notify physician for 2 consecutive aPTTs < 40 or > 60 seconds.
